Software Causing Loss Of Life Or Property?

Are there any specific examples(articles/ news clips) were a software bug/defect caused the loss of life or property. I have been thinking a failure at a nuclear facility or air traffic control system or banking system might be areas where this could happen but can’t find specific examples.
